All exogenous AAS suppress the Hypothalamic-Pituitary-Testicular Axis (HPTA) — the hormonal feedback system that governs endogenous testosterone production. This suppression occurs via two converging mechanisms:
Without LH, Leydig cells in the testes cease testosterone production. Without FSH, Sertoli cells cease spermatogenesis support. Duration and severity of suppression varies by compound:
Clomiphene (Clomid) and Tamoxifen (Nolvadex) — Selective Estrogen Receptor Modulators (SERMs) — block ER at the hypothalamus and pituitary. The brain perceives low estrogen → increases GnRH pulses → increases LH/FSH → testes resume testosterone and sperm production. This is the core HPTA restoration mechanism.
Aromasin (Exemestane) — irreversible aromatase inhibitor — reduces estrogen production by blocking CYP19A1 permanently (per dose), removing the E2-mediated feedback component. Useful when aromatizing AAS have been used.
Anastrozole (Arimidex) — reversible competitive aromatase inhibitor — reduces E2 production without permanent enzyme inactivation. Rapid reversal upon discontinuation.
HCG — acts directly on Leydig cell LH/hCG receptors, stimulating testosterone production independent of the hypothalamic-pituitary axis. Useful for testicular maintenance during suppression or for direct testicular stimulation research.
